INSTRUCTIONS FOR COMPLETING THIS FORM

Students who wish to withdraw from studies or take a leave of absence (hiatus) must complete this form in order to officially withdraw or declare that they are on leave of absence. The withdrawal date on record will align with the submission date for this form.

Non-attendance, email declarations, or declaration to an instructor or other designate at the institution does not constitute an official declaration. This form must be submitted in order to officially withdraw or take a leave of absence from studies.

A withdrawal or leave of absence can impact a student's eligibility for Canada Student Loan.

International students must also consider the impact of a leave of absence has on their permission to live, work, and study in Canada according to the terms of their study permit. A leave of absence can have a significant impact on their eligibility to apply for a Post-Graduate Work Permit once their studies have concluded.

It is important to discuss the impacts of a withdrawal or leave of absence with your Academic Advisor or a Regulated International Student Immigration Advisor (RISIA).

Tuition refunds and material fee reimbursements for students withdrawing from studies once a term has begun will be consistent with the posted refund dates. Students who withdraw completely from the current term before the 100% refund date will be charged a $100 withdrawal fee.
