Students may change their degree program after the Foundation requirements have been met for the intended Degree Major.

Please note that some Degree Majors have a minimum GPA / Grade requirement for entry or continuity in the program. For these programs the student's request will be reviewed by the appropriate faculty for final approval.

Degree Majors that have a GPA or Grade requirement have been indicated wtih a ** notation.
Program of Study (choose only one)

Choose one degree pathway (BA, BDES, or BFA) not more than one as that indicates a Double Degree. A Double Degree declaration can only be approved by the Registrar after consultation with the student on the requirements of fulfilling a Double Degree.

Students who would like to declare a Double Major or Double Degree must first
meet with the Registrar to confirm program requirements and elibility.
Minor Declaration

Students may indicate a Minor declaration. In Self Service a Minor declaration will not be noted, however, a copy of this form will be kept in the student file for reference by the Academic Advisor. Students can select more than one Minor Declaration. Students cannot declare a Minor that is redundant with the Major declaration. (ie Major in Ceramics cannot also include Minor in Ceramics).
