Level Two: Formal Academic Appeal

This form is used for students pursuing the Level Two Procedural Appeal to the Academic Appeal Committee. This form must be submitted to the Assistant Registrar (Office of Student Experience) within five (5) business days after receiving the Level One Academic Appeal decision from their Faculty/Program Chair. Once the submission is received, the Registrar, , or designate, will communicate the meeting date and time to the student and Appeal Advisor. The Academic Appeal Committee meeting will occur with ten (10) days of receipt of this form and the Academic Appeal Committee has five (5) business days to render the decision.

This process requires a $25.00 deposit fee, which will be refunded in case of a favorable decision. Please reach the Office of the Registrar at registrar@nscad.ca to make the deposit once you have filled the form.
